What does SMILE eye surgery cost?

Planning ranges for 2026 put SMILE at $3,500–$5,500 for both eyes, or roughly $1,750–$2,750 per eye. The Refractive Surgery Council reports that most eye surgeons charge about the same for SMILE and LASIK, although some practices charge more because of the laser platform and surgeon training. Your own quote depends on your market, your prescription and what the practice includes.

ProcedureTypical both-eyes range
PRK$3,000–$5,000
LASIK$3,500–$5,500

How SMILE works, in one paragraph

SMILE (small incision lenticule extraction) uses a femtosecond laser to shape a small disc of tissue inside the cornea, which the surgeon removes through a tiny incision. There is no corneal flap. It treats nearsightedness and nearsighted astigmatism, and it is not used for farsightedness. See the full procedure comparison for candidacy and recovery.

Laser platform

SMILE runs on proprietary equipment such as the ZEISS VisuMax system, and the practice has to recover that investment.

Surgeon training and volume

Fewer surgeons perform SMILE than LASIK, and experienced high-volume surgeons often charge more.

Your market

Practices in high-cost metro areas usually quote above the national range, while many Southeast and Midwest markets quote below it. See LASIK cost by city for local context.

What the price includes

Compare follow-up visits, enhancement coverage and medications. Two quotes are only comparable if they cover the same care.

Ask these questions before you book

  • Is the quote for both eyes? Some practices advertise a per-eye price.
  • Which laser platform will be used? Quotes for different technology are not equivalent.
  • Are follow-up visits and enhancements included, and for how long?
  • What is the price for LASIK at the same practice? If your surgeon recommends SMILE over LASIK, ask why.
  • Is the consultation free, and is its cost credited toward the procedure?

Insurance, HSA and FSA

Standard health insurance does not cover SMILE because it is elective. It is a qualified medical expense, so you can pay with HSA or health FSA funds. See whether insurance covers LASIK, SMILE and PRK and how HSA and FSA limits work for 2026, then compare financing options for any remaining balance.

How much does SMILE eye surgery cost?

SMILE eye surgery typically costs $3,500–$5,500 for both eyes in the United States, or roughly $1,750–$2,750 per eye. Your quote depends on the laser platform, the surgeon, your prescription, your market and what the price includes.

Is SMILE more expensive than LASIK?

Usually not by much. Most eye surgeons charge about the same for SMILE and LASIK, according to the Refractive Surgery Council, although some practices charge more because of the laser platform and surgeon training. Compare quotes for the same technology and the same follow-up care.

Does insurance cover SMILE eye surgery?

No. Standard health insurance treats SMILE as elective vision correction. You can usually pay with HSA or FSA funds because it is a qualified medical expense, and some vision plans offer a small discount at participating practices.

Is SMILE worth it compared with LASIK?

Because SMILE usually costs about the same as LASIK, the choice is mostly about fit. It can be a good option if you are nearsighted, have thin corneas or dry eyes, or play contact sports, and your surgeon recommends it. If a practice charges noticeably more for SMILE, ask why they prefer it for you.

Can you finance SMILE eye surgery?

Yes. Many practices offer payment plans or promotional financing, and you can use pre-tax HSA or FSA dollars first and finance only the remainder. Compare the total cost of any plan, not just the monthly payment.
